As we mentioned above, being able to stay at home is imperative for any person that needs dedicated home care. Usually, it can be very stressful to accommodate in a nursing home, whereas home healthcare is a lot easier to handle. Basically, any older person can handle the idea that someone else takes care of their needs within their home.
Optimal Health Services provides home health care and hospice care to patients and families in the comfort of their own homes. Helping people who have experienced illness or injury to remain in their home environment safely and comfortably is our goal. And we have the privilege of providing health care in homes for almost 30 years.
"Quality improvement in public health is the use of a deliberate and defined improvement process, such as Plan-Do-Check-Act, which is focused on activities that are responsive to community needs and improving population health.
The medical home model holds promise as a way to improve health care in America by transforming how primary care is organized and delivered. Building on the work of a large and growing community, the Agency for Healthcare Research and Quality (AHRQ) defines a medical home not simply as a place but as a model of the organization of primary care that delivers the core functions of primary health care.
